Six months
Movement 4 · The Flesh — September 2026
I The figure
On 25 February 2025 a baby with a severe urea cycle deficiency received a gene-editing therapy designed for his specific mutation. In February 2026 the one-year follow-up was published.
About six months passed between diagnosis and first dose. Until then that interval was measured in years, and for these children years meant dying before the treatment existed.
Six months does not prove biology has become fast. It proves that when design costs little, the calendar becomes negotiable.
“What got compressed was not the cell. It was the paperwork around it.”
II Where the time went
| Stage | Before | In this case |
|---|---|---|
| Identifying the variant | weeks | days |
| Designing the edit | months | weeks |
| Manufacturing the clinical batch | months | weeks |
| Approval | years | months |
| Administration | minutes | minutes |
The first four rows moved. The fifth never moves; what happens inside a body takes what it takes, and the follow-up takes longer still.
III Why it does not generalise tomorrow
A therapy designed for one person breaks the entire economic model, the industry's and the public system's at once. There is no phase three trial with a cohort of one patient [and no statistics worth the name], there is no market price for a single batch, and there is no way to spread the development cost across anyone else.
The case worked because three things lined up: a hospital with the capability, a willing regulator and a disease with a clear outcome [comparable without a trial]. Take away any one of the three and it does not happen. All three together are rare.
IV Questions for a case like this [safe]
- How many patients received the therapy? One is a case, not a rate.
- What would have happened without it? With no known natural history there is no comparison.
- How long was the follow-up? A year says little about twenty [and these therapies last as long as the person does].
- Who paid for the development, and who would pay next time?
- Is it published or only announced?
V Risks
The risk in the enthusiasm is treating a case as a platform; you need ten cases at minimum before you can talk about a rate. The opposite risk is treating it as an anecdote, and that would be wrong too. The machinery assembled for that baby stays assembled, with its protocol and its committee: next time it starts from there.
There is also an equity risk worth naming without drama: a bespoke therapy gets made where there is a hospital, a regulator and money, three things that are not evenly distributed.
VI Coda
The design half of biology is falling towards zero. The half that happens inside a body does not move.
All the acceleration in this branch ends in the same place: with somebody who has to say yes, sign fourteen pages, and then wait.
