Exploration

In spinal muscular atrophy a gene is missing and the motor neurons die. The therapy introduces a working copy inside an AAV9 virus that has had everything of its own taken out.

That serotype crosses the blood-brain barrier in the infant, so that an intravenous infusion reaches the cord. The gene does not integrate into the chromosome: it stays floating in the neuron's nucleus.

That is enough because the motor neuron does not divide. If it did divide, the episome would be lost within a few cell generations.

It is given once only in a lifetime, and only once. The body generates antibodies against the capsid and a second dose would be destroyed before arriving.

It was approved in 2019 at 2.1 million dollars. It is a single payment for a product that cannot be repeated, and that argument is what holds the figure up.
