Description

Diazepam, from 1963, was not a chance find: it was deliberately remade to beat chlordiazepoxide, which had indeed turned up in an untested flask. Printed here is the seven-atom ring that pharmacology hardly ever sees and that here is what makes it fit.

Exploration

The GABA-A receptor is a pore that lets chloride in and switches the neuron off. Diazepam does not open it: it sits in a different crevice and makes the GABA that was already there open the pore more times per minute.

It is an amplifier, not a switch, and that is why in an isolated overdose it almost never kills. Without the body's own GABA there is nothing to amplify.

Mixed with alcohol or opiates, which do open channels on their own, that guarantee disappears. Most of the deaths are there.

It is very fat-soluble, so it enters the brain in minutes and then spreads through the body's fat. Its active metabolites last days and build up.

In a few weeks the neuron makes fewer receptors. The dose that worked stops working and withdrawing it produces seizures.
