Exploration

Complement is a cascade of proteins that circulates ready to fire. At the end of the sequence, C5 splits and the fragments assemble a ring that punctures the membrane of whatever is in front.

In this disease the patient's red cells have lost the shield that protected them from that cascade, and the complement itself bursts them one by one.

Eculizumab sticks to C5 and stops it splitting. The cascade reaches that point and halts: the early phases, which serve to mark bacteria, go on working.

The final ring is precisely the only thing that defends against meningococcus. Every treated patient is vaccinated first and many carry permanent antibiotics, and even so the risk is multiplied by a thousand.

For years it was the most expensive drug in the world, around half a million dollars a year, for life.
