Exploration

The earlier antidepressants worked on serotonin, noradrenaline or dopamine, and took weeks. Esketamine acts on glutamate, which is another system, and is noticed within hours.

It blocks the NMDA receptor of certain inhibitory interneurons. On silencing them, the neurons they were holding back fire, glutamate rises all at once and the formation of new synapses is set off.

What improves is not the block but what the brain builds afterwards. The drug vanishes in hours and the effect lasts days.

It is given as a nasal spray in the clinic and the patient stays under observation for two hours, because it produces dissociation and raises blood pressure.

The effect wears out and has to be repeated. Nobody has yet shown what happens to someone who has been receiving it every week for years.
