Description

Ipilimumab, from 2011, does not attack the tumour: it blocks CTLA-4, the brake that stops the immune system doing it. Printed here are its twenty thousand atoms over that brake.

Exploration

CTLA-4 acts earlier than PD-1 and in another place. It does not switch the lymphocyte off in the tumour: it switches it off in the node, during the phase in which it is decided whether that cell is activated or not.

It competes for the same pieces as the switch-on signal and takes them away. It is a brake placed at the door, not on the field.

Blocking it widens which lymphocytes get to be activated. That is why it works on tumours the immune system never got round to considering.

The 2010 trial was the first in the history of metastatic melanoma to lengthen survival. Close to two in ten patients are still alive ten years later, with the disease gone.

It is also the most toxic of its class. Removing the filter at the door lets through lymphocytes against the body itself, and autoimmune colitis and hepatitis are frequent and severe.
