Exploration

For thirty years the amyloid hypothesis held that the plaques in the brain caused Alzheimer's. Trial after trial removed the plaques without the patient improving.

Lecanemab goes not against the mature plaque but against the protofibrils, the intermediate soluble aggregates considered to be the toxic form.

It removes them, and the measured decline slows by around twenty-seven per cent at eighteen months. It improves nobody: it makes the worsening somewhat slower.

Whether that difference is perceptible to a patient or a family is the argument that remains open. The European agency rejected it first and approved it later with restrictions.

Around one in five develops inflammation or cerebral microbleeds, and there are associated deaths. It requires periodic scans and excludes anyone on anticoagulants.
