Description

Paracetamol went on sale in 1955, comes from no plant but from a tar residue, and remains the worst understood of the most taken drugs. Printed here are its nineteen atoms and the margin between the dose and the liver.

Exploration

It comes from no plant. It came out of a coal tar derivative used as a dye, and entered the clinic through a dispensing error in 1886.

It brings fever down and takes pain away without reducing inflammation, a combination no theory fully explains. Where it acts is known better than how.

The liver processes it by two clean routes and a third that produces a toxic metabolite. That surplus is neutralised with glutathione, of which there is a reserve for a normal dose.

Past that reserve, the metabolite attacks the liver cell directly. There is no warning: the damage shows at twenty-four or forty-eight hours, when it is already done.

It is the leading cause of acute liver failure in several countries, and it is sold without prescription at petrol stations.
