Exploration

The T lymphocyte carries on its surface a brake called PD-1. When something fits into it, the cell switches off; it is the mechanism that stops the immune system turning against the body itself.

Many tumours manufacture the piece that fits that brake. They do not hide: they present the credential that orders no attack.

Pembrolizumab sits on top of the brake and leaves it out of reach. The tumour's credential no longer has anywhere to rest and the lymphocyte, which had been switched off, works again.

In 2017 it was approved for the first time according to a characteristic of the tumour and not according to the organ: any cancer with a certain kind of genetic instability falls under the indication. Oncology stopped being organised by place in the body.

Taking away a general brake has the expected cost. Between five and ten per cent develop an autoimmune disease that can be permanent.
