Description

Approved in 2017, the first CAR-T takes cells from the patient, rewrites them outside the body with an emptied virus and gives them back. Printed here are those four steps: from the last one on, the manufactured and the living no longer come apart.

Exploration

The patient's own T lymphocytes are drawn off by apheresis and frozen. In a plant thousands of kilometres away, the gene for a receptor that does not exist in nature is introduced into them with an emptied virus.

That receptor has an antibody's hand against CD19 on the outside and a lymphocyte's activation machinery on the inside. It recognises like an antibody and responds like a cell.

Before giving them back, the patient's marrow has to be emptied with chemotherapy, so the new ones find room. Once inside they multiply on their own and persist for years.

The product is not a batch: it is the patient. Each manufacture is unique, takes weeks, and if the disease advances in that time it no longer arrives in time.

It also destroys the healthy B lymphocytes, which carry the same mark. Whoever responds is left without antibodies of their own and depends on infusions for life.
