Description

Approved in 1998, etanercept does not block the receptor: it manufactures separately a loose copy of that same receptor, with no cell underneath, so that the inflammatory signal sticks to it and gets nowhere. Printed here are those three steps, with the decoy at the end.

The other molecules against TNF are antibodies that chase the signal. Etanercept is not an antibody: it is the receptor itself, copied and released into the blood with no cell underneath.

It was made by joining the external part of the receptor to the tail of an immunoglobulin, which gives it stability and keeps it circulating for a week.

The inflammatory signal meets the receptor it always met, sticks to it, and there the journey ends. There is no cell behind to pass the order on to.

It is a lure, and in that it differs from a blocker. It does not occupy the lock: it hands out false locks.

That difference has clinical consequences. It works in arthritis and does not work in Crohn's disease, where the antibodies do, because there the cell emitting the signal has to be killed and not merely captured.
