Description

Prednisone, from 1955, copies what the adrenal gland makes. Printed here is the structure where you can see that between a sex hormone and an immunosuppressant there is a difference of a few oxygens.

Exploration

Prednisone is not active either. The liver reduces one group and turns it into prednisolone, which is the one that crosses the cell membrane.

Inside it does not look for a surface receptor: it binds to one floating in the cytoplasm, and the pair goes into the nucleus and sits on the DNA. What it switches off is not a signal, it is the genes that make it.

That is why it takes hours to act and why the effect is so broad. There is no way to switch off inflammation without switching off the defence as well.

Put next to testosterone, the structure is almost the same: four rings and a difference of a few oxygens.

The body, receiving it from outside, stops making its own. Withdrawing it abruptly after weeks leaves the patient without either.
