Description

Approved in 1990, amlodipine was among the first drugs designed against a specific structure rather than found by trial: it blocks a calcium channel and holds on to it by its two esters. Printed here is the structure, with the grip marked.

Exploration

The calcium channels in the arterial wall open with each impulse and let in the ion that contracts the vessel. Amlodipine slides into the membrane and blocks them from inside the lipid itself.

The vessel stops squeezing, resistance falls and pressure with it. It does not touch the heart: selectivity for vessel muscle is what separates it from its predecessors.

The two esters hanging off it give it a charge that keeps it anchored in the membrane for hours. The half-life runs from thirty to forty hours.

From there comes its practical advantage: one dose a day, and forgetting one does not undo the effect.

The calcium it keeps out of the vessel also leaves the capillaries for the tissue. That is why ankles swell, and it is the commonest reason for giving up.
