Exploration

HMG-CoA reductase makes cholesterol in the liver in the small hours. Atorvastatin carries a tail that imitates that enzyme's substrate and occupies its place without reacting.

The liver, on noticing the shortfall, does not respond by producing more: it makes more receptors to capture the cholesterol circulating in the blood. The measured drop comes not from what stops being produced but from what is withdrawn.

It arrived fifth into an already occupied market and inside the company itself there was argument about whether releasing it was worth it. The trial that settled it did not measure cholesterol but heart attacks.

It billed some hundred and fifty billion dollars before losing the patent.

Muscle also uses that pathway. Muscle pain is the effect that most often interrupts the treatment.
