Description

Withering published in 1785 the recipe of a Shropshire herbalist with twenty herbs, of which only the foxglove did anything; Sydney Smith isolated digoxin in 1930. Printed here is the margin between the dose and the toxic dose.

Exploration

Digoxin blocks the pump that takes sodium out of the heart muscle cell. Sodium builds up inside, the exchanger that expelled it in return for letting calcium in slows down, and more calcium is left available for contraction.

The result is a stronger, slower beat. The three-step chain has no shortcut: there is no way to strengthen the contraction without first touching the sodium balance.

The problem is the margin. The useful concentration and the one that produces arrhythmia are so close that it is measured in the blood, and even so the poisoning is one of the commonest in hospital.

One of its signs is visual: objects look yellow and haloed, because the same pump is in the retina.

Withering took ten years to establish the dose. He wrote down every case, including the ones he killed.
