Exploration

In 1998 it was observed in a worm that introducing double-stranded RNA silenced the corresponding gene. The finding earned the Nobel eight years later.

The mechanism was there beforehand, as a defence against viruses. The cell cuts the double RNA into pieces of twenty-one pairs, keeps one strand and uses it as a template to destroy every messenger RNA that matches.

A drug of this kind does not block the protein: it stops it ever being made. It acts one step earlier than everything else.

The first was approved in 2018. It is given conjugated to a sugar that the hepatocyte's receptor recognises, and hence almost all the approved ones are liver diseases.

The effect of one injection lasts months. There is one against cholesterol given twice a year, and that changes the biology less than it changes the idea of adherence.
