Description

A conjugate ties a poison too toxic to be given alone to an antibody that carries it as far as the diseased cell; the first of the current generation was approved in 2019. Printed here are the three parts: the antibody, the linker and the payload.

Exploration

There are cell poisons a thousand times more potent than the usual chemotherapy and that cannot be administered, because they would kill the patient before the tumour.

A conjugate ties them to an antibody that recognises a mark on the diseased cell. The antibody binds, the cell swallows it, and inside the acid compartment the linker breaks and releases the payload.

The linker is the critical piece. If it holds too briefly, the poison is released in the blood; if it holds too long, it is not released where it is needed.

The first attempts, in the nineties, failed for that reason and one went as far as being withdrawn from the market. The current generation solved the linker and some drugs work even where the mark is scarce, because the released payload reaches the neighbouring cells.

The toxicity is no longer that of the antibody or of the poison alone, but that of what comes loose along the way.
