Description

Imatinib was approved in 2001 against a protein that exists only in the diseased cells, and turned a fatal leukaemia into a chronic illness. Printed here is the molecule wedged into BCR-ABL.

Exploration

In this leukaemia two chromosomes have swapped a piece and the splice makes an enzyme that exists in no healthy cell and that is switched on always.

Imatinib slots into the cleft where that enzyme lodges ATP and holds it in its inactive shape. It does not destroy it: it leaves it folded so that it cannot close over the substrate.

The target is literally exclusive to the tumour, and that is why years of daily treatment are tolerated. It is not chemotherapy with better aim: it is another thing.

Five-year survival went from around thirty per cent to more than ninety. The disease is not cured, it is turned into a pill a day.

Resistance appears as a change of a single amino acid in the cleft. There are already three generations of molecules for the successive changes.
