Description

Lenvatinib, approved in 2015, inhibits several kinases at once instead of picking one, and that lack of aim is exactly what makes it work. Printed here is the molecule against its multiple targets.

Exploration

The design of a kinase inhibitor normally pursues selectivity: find the one enzyme responsible and touch no other.

Lenvatinib does the opposite. It blocks at once the receptors for vascular growth factor, those for fibroblast factor, PDGFR, KIT and RET, and it does so with a binding mode that lets it enter several different clefts.

A tumour that loses one supply route opens another. Closing only one is teaching it which to use next.

In thyroid cancer refractory to iodine, the response rate passed sixty per cent against little more than one for placebo.

The lack of aim is paid for in full in toxicity: hypertension in most, and a long list of effects that forces the dose down in half the cases.
