Exploration

CD20 is a mark B lymphocytes carry from an intermediate phase of their development until shortly before becoming plasma cells. Neither the stem cells nor the final antibody producers have it.

Rituximab sticks to that mark and does nothing by itself. What it does is point: complement attacks it and natural killer cells recognise it by the antibody's tail.

The drug is a label. The one that destroys is the body.

The choice of CD20 was the choice of the interval: the whole mature B compartment is wiped and the origin is kept, so that at six or twelve months the lineage repopulates on its own.

Approved in 1997, it was the first monoclonal against a cancer. It later turned out to serve for arthritis, multiple sclerosis and several autoimmune diseases as well, by the same procedure.
