Description

Approved in 1998, trastuzumab is of use only if the tumour overexpresses HER2, so it forced the tumour to be analysed before being treated. Printed here are its twenty thousand atoms over that receptor.

Exploration

HER2 is a growth receptor. In a proportion of breast tumours the gene is multiplied and the cell carries a million copies on its surface instead of the usual twenty thousand.

Trastuzumab sticks to an external domain of the receptor, stops it pairing with another and marks the cell so that natural killer cells recognise it.

It is of use only if the receptor is overexpressed. In the rest of tumours there is nothing to bind to, and that is why it forced every tumour to be analysed before deciding the treatment.

That requirement created the figure of the companion diagnostic: a drug approved together with the test that says who to give it to.

The same receptor is in heart muscle, where it takes part in repair. Heart failure is its characteristic toxicity, and it usually reverses on stopping.
