Exploration

A tumour accumulates mutations of its own and some produce protein fragments that exist in no other cell of the body. Those neoantigens are the signature of that tumour and of no other.

The patient's tumour is sequenced, an algorithm predicts which of its mutations will give fragments the immune system can recognise, and a messenger RNA that encodes them is made.

The resulting product is of use to nobody else. There is no batch, no stock and no study of a drug: there is one manufacture per person, in about six weeks.

In melanoma, combined with pembrolizumab, it reduced relapse by around forty per cent against the antibody alone. It is in phase 3.

All the remaining problem is industrial. A therapy that has to be manufactured anew each time has no cost curve that falls with volume.
