Exploration

Vincristine binds to tubulin and stops it assembling. The spindle never forms and the cell halts halfway through division.

It is the exact reverse of paclitaxel: one freezes the assembled tube, the other does not let it be assembled, and both stop the same thing.

Peripheral neurons use microtubules to transport material along the axon, which can be a metre long. Without that transport, the far end is left without supply, and hence the tingling and the loss of strength that begins in the fingers.

The dose is capped at two milligrams per administration, whatever the size of the patient. Above that it gains no efficacy and the neuropathy turns irreversible.

It is intravenous and absolutely never intrathecal. That error, committed dozens of documented times, always kills.
